The Department of Agriculture has evidence that on or about P6C6ITlber 9, 2010 & May 19, 2010 you or your
organization committed the following violation of Federal Regulations:

2.40 (b)(2) Attending Veterinarian and Adequate Veterinary Care - DIRECT - failure to provide adequate veterinary
care that uses the appropriate methods to prevent, control, diagnose, and treat diseases and injuries

3.1 (f) Housing Facilities, General - DIRECT - failure to provide an adequate waste drainage system

3.2(d) Housing Facilities, General - DIRECT - failure to maintain surfaces in contact with dogs impervious to
moisture

3.6(a)(2)(i) Primary Enclosures - DIRECT - repeated failure to construct and maintain primary enclosures so that they
have no sharp points or edges that could injure the animals

3.6{a)(2)(xi) Primary Enclosures - DIRECT - failure to provide primary enclosures that have sufficient space to allow
the animals to sit, and lie in a comfortable, normal position

3.11 (a) Cleaning, Sanitization, Housekeeping and Pest Control - DIRECT - failure to remove excreta and food
waste daily from primary enclosures

3.11(b) Cleaning, Sanitization, Housekeeping, Pest Control - DIRECT - failure to properly sanitize primary
enclosures once every two weeks

3.1 1(c) Cleaning, Sanitization, Housekeeping, and Pest Control - DIRECT - repeated failure to maintain the premises
clean and in good repair

3.1 1(d) Cleaning, Sanitization, Housekeeping, and Pest Control - DIRECT - failure to provide an adequate pest
control program
